This is a bilingual form, so you can fill it in Hindi or English. In some states, the slip may also come in local languages like Tamil, Marathi, Bengali, Gujarati, or Telugu depending on the branch region.

  1. Account Type: At the top, select your account type such as Savings or Current account.
  2. Withdrawal With: Under the “With” section, tick Passbook because most customers withdraw using a passbook. Other options may include OVD (Officially Valid Document) or GCC (Green Channel Counter).
  3. Date and Branch Name: Write your SBI home branch name if it is not already stamped. Then enter the withdrawal date.
  4. Account Holder Name: Write your full name exactly as per bank records or OVD documents. Example: RAMESH KUMAR SHARMA
  5. Account Number: Enter your account number carefully, one digit in each box. SBI account numbers are usually 8 to 11 digits.
  6. Amount in Words and Numbers: In the “Please Pay Self” section, write the amount in words.
    • Example: Five Thousand Only (Then write the same amount in numbers near the ₹ symbol.)
    • Example: ₹5,000/-
  7. PAN Number: If withdrawing ₹50,000 or more using self-cheque, mention your PAN number.
  8. Mobile Number: You can enter your active mobile number for convenience.
  9. Home Branch Name: Mention your home branch name again if required.
  10. Signature : Sign exactly as per your SBI records or cheque book. If you cannot sign, use your thumb impression in the given box.
  11. Back Side Signature: On the reverse side of the form, you also need to put two signatures or thumb impressions for verification.
